## What ME/CFS is

Myalgic encephalomyelitis, also called chronic fatigue syndrome (ME/CFS), is a long-term illness that affects many systems of the body. Its central sign is post-exertional malaise: after physical, mental or emotional effort, even a small one, symptoms get worse, often a day or two later, and can take days or weeks to settle. People also have extreme tiredness that rest does not relieve, sleep that does not refresh, problems with thinking and memory ("brain fog"), pain in the muscles and joints, headaches, and often dizziness on standing.

ME/CFS is a recognised medical illness. It is not laziness, a lack of willpower or "all in the mind", and it is not caused by a person's character. It often starts after a viral infection, such as glandular fever ([**mononucleosis**](https://www.body-hemisync.com/spleen/mononucleosis)), and research points to changes in the immune system, the nervous system and the way the body makes energy. It can be mild or so severe that a person is housebound or bed-bound for years.

There is no single test. A doctor makes the diagnosis by listening to the symptoms and ruling out other causes with blood tests. There is no cure yet, but care can help: learning to manage energy and stay within your limits (pacing), help with sleep, pain and other symptoms, and support from a specialist team. Exercise programmes that push a person to do a little more each time regardless of symptoms are no longer recommended in the UK, because they can make ME/CFS worse.

## The emotional meaning of ME/CFS

None of what follows is a cause of ME/CFS, and none of it means the illness is imagined. Spiritually, the illness raises questions about how you treat yourself, and about the pressure you may have lived under before it began. Take each one as a question you may or may not recognise.

### The moment it began

Spiritually, it helps to look at what you were living through around the time of the first infection. Had you just decided to leave home? Had you lost someone, gone through a break-up, or felt [**rejected**](https://www.body-hemisync.com/emotions/rejection)? An illness can arrive at a moment that was already heavy.

### Success at the price of your dreams

ME/CFS is linked to a strong wish to succeed at everything you undertake. You may have put your dreams and ambitions aside to meet the performance that society demanded of you, or that you demanded of yourself. You may be uncompromising, want to manage on your own and hesitate to ask for help. Under that, there can be a great [**fear**](https://www.body-hemisync.com/emotions/fear) of failure, of life and of responsibility, and a feeling that you cannot live up to what is expected of you.

### A self built for others

You may want very much to be recognised and admired. To get there, you may have shaped another personality that answers what others want, while your real self was set aside. Keeping up that second self takes a great deal of energy.

### Surviving when others did not

Sometimes the illness follows an event such as an accident in which you survived and someone else did not. The [**guilt**](https://www.body-hemisync.com/emotions/guilt) of having survived, the question "why me and not them?", can be heavy enough to wear down all your strength. If this is your story, it deserves to be heard, with a counsellor or a trauma specialist if you wish.

### Living from the head

Spiritually, ME/CFS is linked to living your life "with your head" when it asks to be lived with your heart and your gut. You may insist and push, spending huge amounts of energy in fighting the current when it would be easier to go with it. You may also find it very easy to run yourself down, to belittle yourself and to compare yourself unfavourably with others. A harsh inner voice is draining in itself.

## A gentler way to hold it

- **Learn to love yourself more.** You are the most important person in your own life. Treat yourself as you would treat a friend who is ill.
- **Go with the current.** Notice where you are forcing, and let go of the fight you do not need. In daily life, this can look like pacing: stopping before you reach your limit, not after.
- **Listen to your inner voice.** It can spare you detours that cost energy you do not have.
- **Spend saved energy on what you love.** When there is a little energy to spare, let some of it go to small things that remind you of your own qualities and your taste for life.
- **Turn obligations into choices.** Look at your "musts" and ask which ones you would choose freely, and which you could put down.
- **Let others help.** Asking for help is not failure. It is part of caring for yourself.

## **Questions to ask yourself**

Take your time with these. There are no right answers; simply notice what comes up.

- { "What was I living through around the time I first became ill": "a move, a loss, a break-up, a rejection?" }
- Which dreams and ambitions did I put aside to meet what was expected of me, or what I expected of myself?
- Do I find it hard to ask for help, even now?
- Have I built a version of myself to please others, and where is my real self in that?
- How often do I compare myself with others and come off worse?
- Which of my obligations could become a choice, freely made?

## **When to see a doctor**

**Please seek medical advice if you notice:**

- Extreme tiredness that has lasted several weeks and does not improve with rest, especially if symptoms get worse after even small efforts. A GP can look for other causes and discuss ME/CFS.
- Unexplained weight loss, night sweats, fever, swollen glands, or a new pain in one place. These point towards other illnesses and need checking.
- Chest pain, fainting, or sudden breathlessness. Seek urgent care.
- A low mood, hopelessness, or thoughts that life is not worth living. Living with a long illness is hard, and support is available. Speak to a doctor the same day, or call emergency services if you are in danger.
- Being pushed into an exercise programme that makes you worse. Ask for a specialist ME/CFS team and an energy management plan.
- Do not stop a medicine you have been prescribed for sleep, pain or mood because of anything you read here.
