## What a tooth abscess is

A tooth abscess is a pocket of pus caused by a bacterial infection in or around a tooth. A periapical abscess forms at the tip of the root, usually when bacteria reach the soft centre of the tooth (the pulp) through [**tooth decay**](https://www.body-hemisync.com/mouth-and-teeth/tooth-decay), a crack or old dental work. A periodontal abscess forms in the gum next to the root, often linked to [**gum disease**](https://www.body-hemisync.com/mouth-and-teeth/gum-disease).

The main sign is an intense, throbbing [**toothache**](https://www.body-hemisync.com/mouth-and-teeth/toothache) that can spread to the ear, jaw or neck. The tooth may be sensitive to hot, cold or biting, the gum may be red and swollen with a tender lump, and there can be a bad taste or smell if pus drains. A dentist treats the abscess by draining the pus and removing the source of infection, through root canal treatment or by taking the tooth out. Antibiotics are added if the infection spreads or you are unwell. Without treatment, the infection can spread into the jaw, face, neck or blood, which can be serious.

### A decision that eats you from the inside

When the infection sits in the centre of the tooth, the meaning becomes an irritation that gnaws at you from within. French plays on *dedans*, "inside", and *dent*, "tooth": something is eating you *de-dent*. The decision is there, unmade, and it keeps working on you quietly.

An abscess builds pressure and then bursts. In the same way, feelings that have been held down for a long time may finally erupt. The question is which emotions you have kept under the surface, and for how long.

## **When to see a doctor**

**Please seek medical advice if you notice:**

- Swelling of the face, neck or around the eye, or a high temperature. Contact a dentist or NHS 111 urgently.
- Difficulty swallowing, breathing or opening your mouth, or a swollen tongue or floor of the mouth. Call emergency services or go to A&E, as the infection can spread.
- Feeling very unwell, confused, cold and clammy, or a fast heartbeat with the infection. Get emergency help, as these can be signs of sepsis.
- Severe, throbbing toothache, a bad taste, pus, or a tender lump on the gum. See a dentist promptly. An abscess does not clear up on its own, even if the pain stops.
- If antibiotics are given, take the full course and keep the dental appointment to drain or treat the tooth.
